1. Purpose & Scope
Scaffolds and elevated work platforms are essential on nearly every HCMI church construction project — from exterior masonry and cladding to interior ceiling work in sanctuaries and gymnasiums. Falls from scaffolds and aerial lifts remain one of the leading causes of serious injury and death in Ontario construction. This guide covers the regulatory requirements, safe work practices, and HCMI-specific policies for all scaffold and elevated platform work.
This training applies to every HCMI worker who erects, dismantles, inspects, or works from scaffolding, as well as anyone who operates or works from a boom lift, scissor lift, or other elevating work platform.
2. Legal Requirements
Scaffold and elevated work platform requirements for Ontario construction projects are set out in O. Reg. 213/91 (Construction Projects):
- s.125-141 — Scaffolds: Cover design, erection by a competent person, load ratings, guardrails, planking, access, and inspection requirements for all scaffold types.
- s.142-143 — Elevating Work Platforms: Requirements for scissor lifts, boom lifts, and other self-propelled platforms including operator training, daily inspections, and safe operating procedures.
- s.144-153 — Suspended Access Equipment: Requirements for swing stages, bosun's chairs, and other suspended platforms used on exterior building work.
- s.26-26.9 — Fall Protection: General fall protection requirements that apply whenever a worker may fall 3 metres (10 feet) or more, including guardrails, travel restraint, fall arrest, and safety nets.
- OHSA s.25(1)(c): Employers must ensure equipment is maintained in good condition and complies with applicable regulations.
Regulatory Note: Under O. Reg. 213/91 s.125, a scaffold must be designed by a professional engineer if it is more than 15 metres (50 feet) in height, or if it is a non-standard configuration. The engineer's design drawings must be available on-site.
3. Types of Scaffolding
Frame Scaffold (Most Common)
Pre-fabricated steel frames that stack vertically with cross-bracing. This is the most common scaffold type on HCMI projects. Frames come in standard widths (typically 5 feet) and heights (typically 5 feet per section). Frame scaffolds are economical and quick to erect for straightforward, flat-wall applications such as exterior brick or stone installation on church buildings.
Tube-and-Clamp (Tube-and-Coupler) Scaffold
Individual steel tubes connected with forged steel clamps. This system is highly versatile and can be configured around complex building geometries — ideal for church steeples, curved walls, and irregular facades. Erection requires more skill and time than frame scaffold. All clamps must be tightened to manufacturer-specified torque.
Rolling (Mobile) Scaffold
A scaffold mounted on casters for repositioning. Commonly used for interior work such as ceiling installation, painting, and mechanical/electrical rough-in in church sanctuaries and fellowship halls. O. Reg. 213/91 s.130 requires that castors have functional brakes that are locked whenever workers are on the platform. The height-to-base ratio must not exceed 3:1 unless the scaffold is secured to prevent tipping.
Suspended Scaffold (Swing Stage)
A platform suspended by ropes or cables from roof-mounted outriggers. Used for exterior work on tall church facades where ground-based scaffolding is impractical. O. Reg. 213/91 s.144-153 imposes stringent requirements including engineered rigging, independent safety lines, and operator training. Workers on swing stages must wear a full-body harness connected to an independent lifeline at all times.
4. Erection & Dismantling
O. Reg. 213/91 s.126 requires that scaffolds be erected, altered, and dismantled under the supervision of a competent person. Key requirements:
- Foundation: Scaffolds must be erected on firm, level surfaces. Use base plates and mudsills to distribute loads. Never place scaffold legs on unstable ground, ice, or backfill that has not been properly compacted.
- Plumb and level: Each section must be checked for plumb and level as it is erected. Use spirit levels and plumb bobs. Out-of-plumb scaffolds are prone to collapse.
- Cross-bracing: Install all cross-braces as the scaffold is erected. Never omit bracing to create walk-through openings — use bracing with integrated gates instead.
- Tie-ins: Secure the scaffold to the building structure at intervals specified by the manufacturer or engineer. Typical requirement: every 3 bays horizontally and every 4 metres vertically.
- Planking: Scaffold platforms must be fully planked with no gaps greater than 25 mm (1 inch). Planks must overhang their supports by at least 150 mm (6 inches) but no more than 300 mm (12 inches). Planks must be scaffold-grade lumber or manufactured scaffold decking.
- Dismantling: Remove components from top down. Never pull scaffolding apart from the bottom. Maintain bracing and tie-ins until the final sections are removed.
HCMI Requirement: A green "Scaffold Safe for Use" tag must be attached to every scaffold on HCMI projects before workers may use it. If a scaffold is incomplete, damaged, or under construction, a red "Do Not Use" tag must be displayed. Only the competent person who inspected the scaffold may attach the green tag.
5. Daily Inspection Requirements
O. Reg. 213/91 s.131 requires that a competent person inspect scaffolds before each work shift and after any event that could affect structural integrity (wind storm, impact, alteration). The daily inspection must verify:
- All base plates and mudsills are intact and the scaffold is level
- All cross-braces, tie-ins, and connections are secure
- Planking is complete, in good condition, and properly secured
- Guardrails, mid-rails, and toe boards are in place on all open sides
- Access ladders or stairways are secure and unobstructed
- No visible damage, corrosion, or deformation of any component
- The scaffold is not overloaded beyond its rated capacity
- Green tag is current and displayed
Inspection results must be documented on the HCMI Daily Scaffold Inspection Form. Any deficiency must be corrected before workers use the scaffold, or the scaffold must be tagged "Do Not Use" until repairs are completed.
6. Load Ratings & Capacity
Every scaffold must have a rated load capacity established by the manufacturer or the designing engineer. O. Reg. 213/91 s.127 states that no scaffold shall be loaded beyond its rated capacity.
- Light duty: 25 pounds per square foot (psf) — suitable for painting and light finishing work
- Medium duty: 50 psf — suitable for general construction including bricklaying with limited material storage
- Heavy duty: 75 psf — suitable for masonry with full material storage on the platform
The rated capacity must account for the combined weight of workers, tools, materials, and the scaffold platform itself. On HCMI church projects, brick and stone masonry is common — ensure scaffolds used for masonry are rated for heavy duty and that material is distributed evenly, not concentrated on one section.
7. Guardrails & Fall Protection
O. Reg. 213/91 s.26.3 and s.128 require guardrails on all open sides and ends of scaffold platforms where a worker could fall 2.4 metres (8 feet) or more. The guardrail system must include:
- Top rail: 1,070 mm (42 inches) above the platform, capable of resisting a 200-pound force applied in any direction
- Mid-rail: Approximately midway between the top rail and the platform
- Toe board: A board at least 100 mm (4 inches) high at the platform edge to prevent tools and materials from falling
Where guardrails are not feasible (for example, during erection and dismantling), workers must use a personal fall arrest system — full-body harness, shock-absorbing lanyard, and adequate anchorage point. On HCMI projects, workers erecting or dismantling scaffolds above 3 metres must wear fall arrest at all times.
8. Access & Ladders
O. Reg. 213/91 s.129 requires safe access to all scaffold platforms. Workers must never climb cross-bracing or scaffold frames to reach the platform.
- Built-in ladder: Many frame scaffold systems include integrated ladder rungs. These must be in good condition with no missing rungs.
- Scaffold stairway: For scaffolds used frequently or for extended periods, stair towers provide the safest access and are required by HCMI policy for any scaffold exceeding 6 metres in height.
- Attached ladder: An extension or fixed ladder secured to the scaffold. Must extend at least 900 mm (3 feet) above the platform for safe dismount.
- Access gates: Where ladders pass through scaffold platforms, the opening must be protected by a self-closing gate or equivalent to prevent falls.
9. Aerial Lifts — Boom & Scissor
Aerial lifts (elevating work platforms) are regulated under O. Reg. 213/91 s.142-143 and CSA B354 standards.
Operator Requirements
- Operators must be trained and authorized by the employer before operating any aerial lift. Training must cover the specific type and model of lift being used.
- Operators must conduct a pre-use inspection following the manufacturer's checklist before each shift.
- Only workers trained in the safe operation of the specific lift may operate the controls — this includes both platform and ground controls.
Boom Lifts (Articulating & Telescoping)
- Workers in the platform must wear a full-body harness with a lanyard attached to the manufacturer's designated anchorage point inside the basket. This is mandatory — boom lifts can catapult occupants if the boom contacts an obstruction.
- Set up on firm, level ground. Use outriggers where equipped. Maintain safe distances from overhead power lines (minimum 3 metres from lines under 750V, greater distances for higher voltages per O. Reg. 213/91 s.188).
- Never exceed the rated platform capacity. Account for the weight of all occupants, tools, and materials.
Scissor Lifts
- Guardrails on scissor lift platforms provide primary fall protection. A harness is not required on scissor lifts unless the manufacturer specifies it or the worker must lean beyond the guardrails.
- Never drive a scissor lift with the platform in the elevated position unless the manufacturer allows it and the ground surface is firm and level.
- Watch for overhead obstructions, especially in church sanctuaries with suspended lighting, sound systems, and decorative elements.
- Ensure wheel chocks or brakes are engaged when the platform is elevated and the lift is stationary.
Regulatory Note: O. Reg. 213/91 s.142(1) requires that an elevating work platform be operated and maintained in accordance with the manufacturer's written instructions. A copy of the operator manual must be available on-site for each lift in use.
Daily Aerial Lift Inspection
Before each shift, the operator must inspect and verify:
- All controls function properly (platform and ground controls)
- Emergency lowering system is functional
- Hydraulic system has no visible leaks
- Tires/tracks are in good condition with proper inflation
- Guardrails and gates are secure and functional
- All safety decals and load rating placards are legible
- Horn, lights, and motion alarm are operational
- Outriggers (if equipped) extend and retract properly
10. HCMI Scaffold & Lift Policy
- Competent Person: A designated competent person must supervise all scaffold erection and dismantling on HCMI sites. This person must have documented training and experience in scaffold construction.
- Tagging System: All scaffolds must display a green "Safe for Use" or red "Do Not Use" tag. The tag must include the date, the inspector's name, and the rated load capacity. Tags are available from the HCMI site trailer.
- Stair Tower Requirement: Scaffolds exceeding 6 metres in height on HCMI projects must be equipped with a stair tower for access. Ladder-only access is permitted below 6 metres only.
- Aerial Lift Operator Card: All aerial lift operators on HCMI sites must carry a valid operator training card for the type of lift they are operating. Training records are verified by the site superintendent before first use.
- Overhead Power Lines: No scaffold or aerial lift shall be set up or operated within 6 metres of an energized overhead power line without a written safe work procedure approved by the HCMI Project Manager and the utility company.
- Wind Policy: Scaffold work is suspended when sustained wind speeds exceed 40 km/h or gusts exceed 50 km/h. Aerial lift work is suspended per manufacturer limits, typically 28-45 km/h depending on the model.
- Incident Reporting: Any scaffold collapse, near-miss, or aerial lift malfunction must be reported to the HCMI Safety Coordinator immediately and documented on the HCMI Incident Report Form.
11. Daily Scaffold Inspection Checklist
Complete before each work shift:
- Base plates and mudsills stable and on firm ground
- Scaffold is plumb and level
- All cross-braces installed and secured
- Tie-ins to the building are in place at required intervals
- Planking is complete, in good condition, and properly overlapped/secured
- Guardrails (top rail, mid-rail, toe board) on all open sides and ends
- Access ladders or stairway secure and unobstructed
- Access gates in place and functional at ladder openings
- No visible damage, bending, corrosion, or missing components
- Load on platform does not exceed rated capacity
- Materials stored evenly — no concentrated point loads
- Green "Safe for Use" tag displayed with current date and inspector signature
- Area below scaffold barricaded or signed to protect workers from falling objects
- Weather conditions assessed — wind speed within acceptable limits
Remember: If you observe any scaffold deficiency, do not use the scaffold. Report it to the competent person immediately. Working from a deficient scaffold is a violation of the OHSA and puts your life at risk.
